two months after my fiance came to us i am doing AOS for her, in JFK airport they toke all medical and vacinne forms and cds . what should i do for form i-693-report of medical?

They took the medical forms because they were supposed to and your fiance (should be WIFE before you file for AOS) was supposed to have been given a copy of the immunisation record by her doctor or at least (if given the chance) taken a copy for her records.

So to answer your question, she will need to contact the doctor in her home country to gain a copy of the immunisation record. Failing that she will need to have a blood test done to prove what immunisations she had (which you need to pay for) and then taking the results from that, go to a CS to have it transcribed into the form, and then any further shots will need to be given.
