Two (2) valid identification cards (IDs) with photograph;

Duly completed guidance and counseling form;

If married, certified true copy and photocopy of marriage contract on security paper from the National Statistics Office, or Local Civil Registry Offices; or original and photocopy of marriage contract duly authenticated by the Philippine Embassy/ Consulate (if married abroad)

Other documents as may be required by the counselors; and

Payment of P250.00 counseling fee.

This is all that the CFO website says to bring, but the "other documents as may be required is where they get you...

Do you know if they will need to bring any Passport photos ? Also if she brings her Birth Cert and Cenomar will they be keeping it ?

They told her that the photos weren't large enough. The ones we had were passport, NBI, Police Clearance size. They wanted a full 2"x2" photo she said. I think I read that on the USEM website but I never measured the photos we had taken. I assumed that the size we had was what was required based on where we had them taken and that they were ok with NBI, Police and for her passport. She had the new ones done right there close to St. Lukes somewhere. I'll ask her tonight :thumbs:

She had all the information with her that she'll be taking to the interview teamster. I didn't get to find out a lot from her, been long days there, but I don't think she was even asked for much. Also, when she got to the medical yesterday, all of the passport photos we had taken there for this process, were unacceptable to St. Lukes. They wanted 2x2 passport style photos - wait, wut?? THAT'S WHAT SHE HAD :rofl: We had photos done at a professional place that does photos for immigration, white background, hair off the ears, black suit jacket, etc. Luckily there's a place right there close where she could go get "the right one" done quickly. Oh, and yes it's the same photo that's on her NBI, PCC and Passport :blink:

FYI on requirements for US passport style photos...I think it's in the guides too.

2x2 in size

Make sure photo presents full head from top of hair to bottom of chin; height of head should measure 1 inch to 13⁄8 inch (25 mm to 35 mm)

Center head within frame

Make sure eye level is between 11⁄8 inch and 13⁄8 inch (28 mm and 35 mm) from bottom of photo

Hey Kevin, Philippine passport size is different. It's not 2x2. Wilma went to the Kodak shop early on in the process and got passport photos...but didn't specify 2x2. They gave her PI style...which has blue background and is smaller size. When I got there and looked at them, I told her they were wrong and we went back and took US Passport style photos. It was really cheap out there, I think 150 pesos for 10 pics.

I'm pretty sure Wilma didn't bring any passport photos for her CFO. They take your electronic pic and put it on your certificate. Here's their website, with what's required for the interview.

I don't know as they will ask, but I told my wife to have extras because every time she turns around, she will be asked for more, ie at St Luke's, and the Embassy...so she is going to get a crapload made, and carry em in her purse in case she is asked.

They shouldn't keep the paperwork. I think they just want to verify everything is legal, and correct.

Hi Johnny, yeh we had some of both styles taken. The lady doing our photography was very knowledgeable about the photo requirements and even had the latest government requirements that she's printed from one of the websites (apparently there's ONE set that's newer though..lol). That's why I was surprised that St. Lukes wanted something different. You're right too, they're dirt cheap compared to here! We had about 25 with the blue background taken and close to a million (ok, 30 or so) with the white background and I was shocked at how cheap they were. Yeh, things are lots cheaper there, but it was stupid cheap :):thumbs:

On a side note, I'm glad all Filipinas aren't so picky over 1/4 of an inch otherwise I'd be screwed ;) ..just sayin.

My girl did the CFO seminar right after her St Luke's medical. Without her passport because St Luke's kept her passport. She did it at PRISM CFO and was done in about 3 hrs. She's going get her sticker today. Even if you take the Seminar at PRISM or at St Mary's....you still must go to another office for your sticker. My girl took it in Manila because she lives in Cebu and their CFO lasts all day, from 8 am - 4 pm.

No need for appointment at PRISM. Just show up and have ID and some of the same documents needed for interview (NBI, CENOMAR, NSO Birth Certificate, etc).

Hey Johnny, do you know if it possible to do the CFO seminar in manila and later pick up the sticker in Cebu?

Yeah, that's what my girl is doing right now. She's did her seminar at PRISM in Manila and is getting her sticker today at the CFO Cebu Desk in Cebu City.
