My fiancé has a K-1 visa and is applying for AOS. He had his medical exam in France and has DS 3025, the report of his vaccinations that the doctor completed. Does he need to fill out any parts of the I-693 at all or is the DS 3025 sufficient? Thank you!

The DS-3025 is sufficient if it is complete for vaccination history and a copy of the DS-3025 was included in the POE packet, otherwise an I-693 Part II, Item 5 needs be prepared by a civil Surgeon and submitted with the I-485.

Ok, great. I believe it is complete. How do I know if it was in my fiancé's POE packet?

So do I need to visit a civil surgeon and do the I-693 anyway, or is it overkill?

or you submit what you have and wait for them to request it, if needed....

Follow the VJ AOS Guides:

Have a CS transcribe the DS-3025 information onto the I-693 and then submit that sealed in an envelope.

I'm also from K1. I have DS-3025, what i did is I filled out first part of the I-693 (don't sign it) and have the rest taken care of by the USCIS designated civil surgeon. I didn't go for another medical exam. The CS will check the vaccination worksheet if it is complete, then if it is, he would transfer the vaccination records on I-693, have your fiance sign it then seal it an envelope with a note "DO NOT OPEN, FOR USCIS USE ONLY".

Hope you find a Civi Surgeon which doesn't charge so much soon! Goodluck! :P
