Hi everyone, I'll be moving to NJ on the 31st, this month and need to get this sorted too. But I have a few questions, if anyone wouldnt mind answering. Thank you so much! All this is a minefield!

•I don't have the DS-3025, but I was given a copy of the worksheet at my medical. Will this be a problem? If you don't have the actual DS-3025, will the worksheet and your records suffice?

•I am missing TDAP & MMR. Would you all advise I have this done now (in the UK) and if so, will the civil surgeon accept a letter from my doctor saying I've had these two vaccines after my medical?

•Or should I wait and get it all done in the US by the civil surgeon? Money is really tight in our case so I'm hoping I can get tham done here for free.

•If you get your shots done in the US, should they be done before or after marriage?

•Can you obtain a Civil Surgeon outside of your state? - There are not many recommended CS's in NJ, but a few in NYC. Would this be ok?

The medical exam is in the brown envelope. Since your immunizations are not complete when you arrive in the US you will need to have a civil surgeon (from anywhere in the US) fill out and sign the vaccination record so you can submit it with the AOS papers.

You can obtain your immunizations anywhere you like, if you can do it in the UK for low cost that is great, in the US I recommend at the health department as they are fairly low cost. You do not have to receive the immunizations from the civil surgeon but you do need to have a legible vaccination record for him/her to transcribe to the official form.
