hi another question

i am having my ex employer be my co-sponser and when he is filling it out when it asks who his he doing this for he should put my fiancee?? and also he needs to list his wife and kids right as people that depend on him right??? also does he need to add my name anywhere??? just want to make sure we do this right!!
