What if my husband could not recall his ex wives date of birth can we leave it blank or is he really need to write that one? Hopefully this will be our second time to file. There is a question on the form "have you ever filed a petition for this and if yes give name, place and date of filling and result?" ------ "Due to unfortunate problems we canceled out the appointments" Would this be enough reason? Another thing i dont think my husband could recall the day we filed the papers and me either but that was around feb 2007, is feb 2007 ok? But we got the receipt or the approval notice. Really need help for this one. Thank in advance!!!
