Hi guys,

I just got my Packet 3 and am filling out the IV15K & Checklist and I need help.

The form states as follows:

You are required to furnish the original, or a certified copy, of each document plus a photocopy. It is advisable to obtain the necessary photocopies in duplicate, as this will enable you to provide identical copies in the event the first set is lost or damaged. Please do not mail any documents to the Embassy. You will be required to furnish all documents on the day of your visa interview. Note: If more than 4 months has passed since the petition was approved, you will be required to furnish a notarized statement of your intent to marry within 90 days of your arrival in the U.S. Please fill out the information below and sign.

□ Passports □ Birth Certificates □ Adoption Certificates □ Military records □ Court & Prison Records □ Photographs □ Evidence of Support □ Marriage Certificates □ Divorce Certificates □ Notarized Statement □ Deed Poll □ Police certificates: Please list the countries for which you have obtained police certificates _______________________ ______________________ _______________________ _______________________

My question is do I tick notarized statement or not?

I do not know if it is refering to those out of the 4 months approval date or something else.

Please help!

that approval date is the NOA-2 approval date. so with an NOA-2 date of 2010-03-17, it's not been 4 months since the NOA-2 approval date.
