I think I screwed up ROYALLY

When I was getting ready to send out the I-485 application and had a question regarding the vaccination supplement I actually called INS and they told me I did not need to complete due to the fact that since all required medicals and vaccinations records were in order and provided to the INS officer at the port of entry and based on Argentina's Embassy instructions.

NOW I am reading that is required in order to be approved.... I am concerned because we have no copies of anything that was in that envelope BECAUSE THEY TOLD US not to open...

What do I do now??? They are requesting the form however we provided everything to NY and have no way to get a copy....

Can my husband be vaccinated and have another medical test here? Is their a way to get a copy from NY? My case was transfered to California in early July....

Did they give you a copy of the "Vaccination Worksheet" at the medical exam in Argentina? If so, you can take the worksheet into a civil surgeon in the U.S. and they will copy the information from it onto the Vaccination Supplement. The Vaccination Supplement is all you need to send with your AOS application as long as the medical exam for the K-1 Visa was less than one year from the date you send your AOS forms in.

I had just one page copy of my vaccineation ("Vaccination Worksheet") that i got from the hospital and I sent that with my AOS and that's all they want.. (didnt have any copy of anyhting in the envelope either) And in my AOS i said that I already got the vaccine thing from my K-1. and they didnt send me any request for it.

Fortunately my husband was given a copy of all the paperwork from his K-1 medical examination, including the vaccination worksheet. If your husband did not get a copy of anything, then I recommend trying to contact the panel physician who did his medical examination to see if they can send (or fax) a copy of the vaccination record to you, if they still have it on file. If that doesn't work, then I've heard that doctors can test your blood to see which vaccinations you have had. So that would be another option -- have him get the blood test, get any vaccinations he is missing, then have a civil surgeon record everything on I-693A.

But all of this may be a bit premature -- your AOS was transferred to CSC, and so far you haven't received an RFE. It is entirely possible that they will approve you as is -- presumably they either have your husband's medical exam records that were turned over at the POE or have confirmed their existence. So for now maybe you should hang tight and see what happens. (But it wouldn't hurt to contact that panel physician and see if he/she still has copies of your husbands medical exam paperwork!)
