For the DS-156 Nonimmigrant Visa Application, question #40 is "Application Prepared By." Is there any detriment in the petitioner (me) filling out that portion, should I put NONE, or N/A? Just want to be safe and not sorry. By the by, we are preparing for our interview with the Manila Embassy in the Philippines.

Hi, all i knew was regarding to filling-up forms prior to Interview like forms Ds-156,157, 156K.... is your fiance's responsibilities or job to do it.. so she need to sign-in to #41 , bcos he is the non-iimigrant applicant.

#41 is only to be fill-up , if only u used attorney to do the processing ... and/or u have K2 with you in application and ur issuing K2 same form and mother shud sign on that #.

You can just leave it blank if you are filling out the form yourself. That would only be used if an attorney or parent was filling in the form.

The form is expected to have been filled out by the visa applicant. If someone else (such as the petitioner) fills it out for them, then that someone else should be identified in question # 40. That's what the question is there for.

#41 is only to be fill-up , if only u used attorney to do the processing ... and/or u have K2 with you in application and ur issuing K2 same form and mother shud sign on that #.

You can just leave it blank if you are filling out the form yourself. That would only be used if an attorney or parent was filling in the form.

During my interview at US Embassy, I leave it blank, the US embassy personnel who collected the DS forms before going to Windows X let me fill-out the question #40.

I filled out a copy and sent it to my fiance so she can just fill out the one she gets in packet 3. but has all the answers.. hahaha uhhh ohh that is cheating.. haah ok .. well anyways..

May 1, 2006 - Submitted I-129F (Overnight) NSC

May 2, 2006 - NOA1

June 1, 2006 - Transferred to CSC

June 14, 2006 - Notice from CSC it was transferred

June 30, 2006 - Received IMBRA RFE (CSC)

July 5, 2006 - Touched (RFE Received)

July 31, 2006 - APPROVED

August 5, 2006 Physical NOA2

August 15, 2006 NVC Received and Sent

August 22, 2006 AIT sent Packet 3

August 22, 2006 Packet 3 got lost in the mail... sending another.. :( :( :(

October 27, 2006 Interview

As per to uling's question, he wants to fill-up the form in behalf of his fiance ... i only explain it in a way, just like in my case as K1/K2 as the purpose of that #41 ... dats the purpose of the form is for the non-immigrant application so the dat question will be blank or if there's a child accompany cud sign dat # in behalf of the minor child.

And in some using attorney, that wer d attorney do sign. basically ive said, on my own "opinion" .. dats why is stress "i ONLY knew" not everybody knew, im not setting limitation.

I will honestly im not word by word know everything about immigration laws, but im just giving 1 out of 100 possible answer to his question.... everybody here like to help in a little way even not much of info's needed perfectly...... and everybody here also seeking help as also like me. to top it all ... im just sharing ... if u find it wrongly .... i cudn't do anything about it.

I one of 1,000 VJ here asked ur valuable opinion bcos ur the expert. So is my reason reasonable enough?
