Hi Guys,

I have received my packet 3.0 from US Consulate, Mumbai 1 month back. Now, am planning to send packet 3. I have few questions.

A) What documents I have to attach with Packet 3.0 when mailing ?

•Form DS 2001 and form DS-230 Part 1

•Check list filled

•DSL-choice of agent

Any other documents do I need to attach at this moment ?

B) It seems, I have to prepare I-864 for myself. I think, based on povery guide lines, my existing USD savings should be enough.

However, am working as expat in Mumbai, India for US Financial firm since last 18 months. Planning to stay till March 31st, 2011.

Do I need to collect letter from employer stating my salary after relocating back to New Yrok ? Is it required ? Does it help in

consulate as a supporting document ? I am planning to submit 3 yeas tax returns and 5 yrs of tax transcripts. Upto 2008, W2 amount is

decent eough. I have 1 bank a/c still running with minimum balance. However, in my brokerage a/c, amount is good enough. Can I print

them from website and submite ?

Do I need to notarize these documents as most of them are printed from website ( i.e please note, am located in Mumbai, India )

C) Do I need to notarize I-864 ?

D) Which address I have to mention in I-864 ? US Address or India address ?

If I mention 'US Address', they may say "am currently residing in Mumbai since 2 yrs....and may ask more questions about the address".

Is it ok if I put India address ? At the time of DCF filing, they didn't say anything as am on 'E' visa status.

E) Coming to medical examination, is it needs to be done after getting the interview letter ?

Please let me know your opinions/suggestions.

I-864 does NOT need to be notarized.
