Confuse on how to complete mum's petition

My mum and dad have been married for over 35 years but didn't have marriage license. I have had relatives (elders to my mum and dad) sign affidavit affriming mum and dad were traditionally married since 1974. Now mum and Dad decided to initiate a civil marriage license which they got yeaterday 4/9/2010. I am filing for my mum first.

Now the confusion is, what should I fill as date and place of marriage? Go with the affidavit, or the date of civil registry?

Any help will be kindly appreciated.
