Ukraine specific DS-156 question

The internal passport lists the MP#. This is printed on the internal passport itself. Yes they do have a separate paper that also lists a number as well to compliment the passport. I guess for security. Not sure if someone can clarify what the correct number would be. I assume that the MP# is how the internal passport is linked to the system. Key words...Assume. I also need some clarification on this as well.

That is the number we've decided to put on DS-156

Hey everyone.

Just a quick question. On DS-156 when it asks about the passport's issuing city, did you put anything at all?

Since there's no mention about the issuing city in Daria's passport I'm not sure whether to leave it blank or not.

Also, for State/Province, did you put ".... oblast" or left it blank as well?

Issuing city - Kharkiv, State/Province - left it blank (for Kyiv, same for Kharkiv I think)

Another thing: national ID#. There's a number in her international passport, "personal#" is what it's called.

Should I use that??

Yes. I filled DS-156k this way for my B1/B2 last year and it went ok.

Consulate : Kiev, Ukraine

27 Oct 2009 I-129F Sent

13 Nov 2009 I-129F NOA1

24 Feb 2010 I-129F NOA2

01 Mar 2010 NVC received

03 Mar 2010 NVC sent

05 Mar 2010 Consulate received

08 Apr 2010 Interview scheduled

01 Apr 2010 Call from the embassy in Kiev - case sent back to NVC (lost 3d page of I-129F), interview cancelled

I'm just going to put "Karkov Oblast" and the number she found (it's on a separate piece of paper too that is given to them)

I have those papers for Alla and the boys also and it is the same number as in the internal passport. You are good to go.
