Help Filling Out I-129F and G-325A

I'm a little confused on how to fill out the names in the I-129F and G-325A because Russian and Ukrainian names are different than what I'm used to.

My fiancee is Ukrainian and the way she says her name is Morozova Kseniya Sergeevna. That is Family Name, First Name, Dad's Name

So my questions:

What do I put for the Middle Name?

On the G-325A, it first asks for her Family Name, then directly underneath that, it asks for All Other Names Used (including names from previous marriage). She is divorced and still retains her last name from that marriage. So which names should I put as her Family Name and which for All Other Names Used?

Lastly, what do I put for File Number at the top right of the G-325A form?

She doesn't have one, so you would either put "N/A" or "NONE".

On the G-325A, it first asks for her Family Name, then directly underneath that, it asks for All Other Names Used (including names from previous marriage). She is divorced and still retains her last name from that marriage. So which names should I put as her Family Name and which for All Other Names Used?

You would put her complete family name for the first part (without the divorced name) and in the "other" section, just list the previous married last name.

Lastly, what do I put for File Number at the top right of the G-325A form?

If she was assigned an alien number - you would put it there - she probably does not have one at this point.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

I put the form of her Dad's Name as her Middle Name. (Shouldn't that be spelled Sergievna?)

On the G-325A, it first asks for her Family Name, then directly underneath that, it asks for All Other Names Used (including names from previous marriage). She is divorced and still retains her last name from that marriage. So which names should I put as her Family Name and which for All Other Names Used?

For other Names then she would put her name using her Maiden Name.

Lastly, what do I put for File Number at the top right of the G-325A form?

Leave it blank. Follow the Example Forms

Phil (Lockport, near Chicago) and Alla (Lobnya, near Moscow)

As of Dec 7, 2009, now Zero miles apart (literally)!

Whenever a form asks to provide a middle name, I put down my patronymic (dad's name), which in your case would be Sergeevna. The main name should be the legal name, as it shows in the passport. All other names are just that :) I would also advise to make sure you write the names the same on all forms. My name has been spelled differently on passports and visas in the past. You don't want another headache trying to prove that you really are who you say you are :)

03/05/2009- back to Russia for 2-year HRR

02/17/2010- I-130 sent to USCIS

03/01/2010- NOA1

06/02/2010- NOA2

06/09/2010- our case is at NVC!

06/12/2010- receive DS-3032 and I-864 bill

06/19/2010- I-864 bill is paid

06/25/2010- mailed DS-3032 to NVC

07/15/2010- received Immigrant Visa Application Processing Fee Bill Invoice

07/20/2010- Immigrant Visa Application Processing Fee is paid (check cleared)

09/20/2010- call NVC and find out that we can start work on I864 and DS230 packets

02/12/2011- AOS (I-864) packet is mailed to NVC

02/14/2011- NVC receives AOS (I-864) packet

03/06/2011- YAY! Finally, the 2 year HRR is fulfilled![/font]

03/07/2011- IV packet is mailed to NVC

04/07/2011- receive a letter from NVC requesting detailed information regarding the circumstances of my 1999 J-1 visa

04/09/2011- we mail the requested info to NVC along with proof that 2-year HRR has been fulfilled

04/30/2011- Check with NVC- get message "Your case was completed on April 27th. Please allow 8 weeks for further instructions"

05/17/2011- receive notification from NVC: interview scheduled for June 1st!

Middle name would be Sergeevna.

For my fiance I put: Mikhailovna. USCIS (and now NVC) just cut it down to M. and it's going to stay like that.

Same as my middle name. All that is left of it is L.

Okay, great I appreciate everyone's help! So for the middle the votes are unanimous, but we have a few different opinions on what the Family Name should be.

Using her legal name as shown on passports and such (in this case her ex-husband's last name) for Family Name makes good sense to me (after all they were a family at one time right?), but perhaps others can chime in to confirm this for me since I sure don't want to make a mistake and delay the process or even ruin our chances. It's a very important matter!

Use whatever is in her official documents and then, under "other names used" include her maiden name and any other names she went by

Have you two thought of just doing all the legal work to change her name back to maiden? I'm not sure what goes into that process, how much it costs, or how long it takes, but maybe it's worth looking into. I was concerned with the name thing too. I didn't even take my husbands name when we got married. I'm waiting till visa processing is over to do that. The way I'm thinking the less things you give USCIS/NVC to digest the better (and quicker) it's for you.

Hmm, I hadn't really thought of it. Wouldn't doing that just create another document that I would need to provide? Since she used her husband's last name at one time I would need to include that in the 'Other Names' section but then wouldn't I then need to show proof of her changing her name back to the maiden name?
