This get's a little convoluted, but the poverty guidelines are calculated annually by applying a regionally adjusted formula to the poverty threshold. The poverty threshold is calculated by applying an age and household size adjusted formula to the Consumer Price Index. The CPI is calculated by applying a formula to the average retail cost of specific goods and services. The CPI generally does not include the cost of health insurance, though it does include the cost of medical goods and services that health insurance might pay for.

All that said, the poverty guidelines for any year are always based on the previous year's data. That means that any revised guidelines for 2010 are going to be based on 2009 data. If the health care law remains substantially the same as it's current form, there's likely to be a ripple effect in the cost of medical services included in the CPI, but this isn't likely to be significant until the major changes begin to take effect in 2014, so the first bump in the poverty guidelines would be reflected in the numbers for 2015. Depending on whose estimates you believe, the average cost of medical goods and services will either go up or down.
