Police check form filling out question

My fiance is doing his paperwork today, and is at the Canadian police check, form, Consent for Disclosure of Criminal Record Information.

In part 2 of the form, it has boxes, Full name, Title, Name of Organization, Address, City, Province, Postal Code.

Does he just answer the name and address (etc) part and ignore the rest, since he is just himself and not doing this for a company or whatever? I'm sorry if this is obvious. >.<

On the other side, the form, Consent for a criminal record check for a sexual offence for which a pardon has been granted or issued, reason for consent section, It has, description of the paid or volunteer position, name of the person or organization, and details regarding the children or vulnerable person(s)

Which he put N/A into all of them. Is this okay?

This is his first paperwork of the process, and he asked me to ask.

it sounds like you're filling out the wrong form. I believe that one is only used if your fiance has a criminal record (which I'm assuming he doesn't). In the instructions package, it says to contact your local police department. As long as they use the RCMP's database to search using his name and birthday, that's all they need.

In Toronto, it's referred to as a "clearance letter". Check with the local police department instead.

Hmm, a former member of the RCMP said they were the right forms. :unsure:

This is what the RCMP gave him in Westbank, and told him to turn it in at another RCMP in Kelowna. I think thats weird but what do I know.

Oh and no, no criminal history of any kind. He has no police there, just RCMP.

this is so confusing, but here's what I found. Your fiance needs to get a Certified Criminal Record Check, not a Consent for Discolsure of Criminal Records. They are two different things

maybe it would be easier for him to go to the nearest town that has an actual police department? If your local RCMP doesn't have fingerprinting technology, he'd have to get it done by someone else, and it says results can take a long time. if you read the instructions, it says you have to indicate your reason for the check, one of which is immigration. short of going to a major city and getting it done at the police station there, that seems to be the only way to get the document. good luck.

But he isn't supposed to need fingerprinting. Because he has no criminal history, he just needs the form. His RCMP says this was the form he needed, for immigration. They are a very small unit I guess. They can't preform the check, so they sent him to Kelowna for that. I appreciate your help, I'm just hearing two different things, both from his RCMP, and other posters on the forum.

Anyone in the Westbank/Kelowna area able to shed some light on this?

He doesn't need fingerprints and a criminal check on fingerprints done. He just needs a Canada wide name search to see if there is any sort of criminal record listed. You are expecting it to come back and say 'no criminal records were found'.

You could have him check again tomorrow and see if they have another form that just lists his name and various addresses he has lived at that will produce a Canada Wide name check. He could continue to fill out the form he has and bring it back with him to ask them if this is the right form or not. If they say that is the correct form, then fine. If not, then they may be able to have the other available. Have him explain that he needs a non-fingerprint criminal record police check from across Canada for immigration purposes as requested by the US Immigration service. That may be more successful in getting reassurance - and possibly re-issuance :P - of the correct form.

I think you were referring to me above...I still work for the RCMP (just not in a detachment anymore). :) I can guarantee you 100% you have the correct forms. Leave Part 2 blank or he can put in his own information. They will do a Canada-wide name check with that form. If they told you to bring it somewhere else, do it, don't question it. My ex-uncle-in-law works at West Bank Detachment...small world! haha

I tried to edit my above post but it was too late.

The Consent for Disclosure blah blah form is a standard additional form that generally gets given out to applicants seeking regular criminal record checks. Your fiance doesn't really need to do that form but it doesn't hurt. All it means is that the RCMP will search CPIC to see if your fiance has received a pardon for any sex-related offences. It comes standard with regular record checks because most people get CR checks for volunteer stuff where the sex-related search is important. Standard practice...just fill it out.
