I recently got married on a K1 Visa and am now in the process of sorting out my AOS. However, I seem to be experiencing some problems with finding a doctor who can sign off the I-693A form without charging me $$$'s. I had everything done in the UK before I came over and even have the DS-3025 form on me. All I need is the doctor to transfer this information onto the vaccination supplement form but everywhere I call they insist that I need blood tests and this and that. If anyone can recommend a doctor in the Los Angeles area I'd be really grateful.

Some people have had success by just sending the DS-3025 or a copy, rather than getting the I-693A done... you sent that with your AOS and risk a RFE but as I say some people have got their greencard and only sent a copy of the DS-3025....

I had my AOS interview today and we were RFE'd at the interview for not having the immunisation chart on the correct form. We went straight to a civil surgeon office and paid $75 for them to transfer my immunisations to the correct form which we then went and posted in the envelope we had been provided during our interview. All of my immunisations had been done in Australia and I thought they were on the correct form already as it was a form given to me from the Immigration people in Sydney Australia. Anyway I didn't have to have blood tests or anything else. I didn't even see the doctors face for the $75.

How far are you from Santa Ana, CA? We had my wife's I-693A completed by a doctor in Santa Ana. I thought his charges were reasonable.

Thanks for all the quick responses and information. I knew I'd get help here. I will check into some of these options this week.

I didnt have the I693A form either. I just sent in a copy of the DS-3025, my receipts from the doctors showing I paid for the vaccinations and the sticker with the I.D number from the shot packet that the nurse stuck on a piece of paper for me so I could prove that I had had it.

I didnt receive an RFE, nor did I get asked anything about it at my interview on friday just gone.

Maybe that will work for you too if you have all of that evidence??

The I-693A should be filed together with your I-485.

We didn't file Atif's with the AOS forms.....it says that the I-693-A can be filed with the form...not Must be filed.....but we will have it done by the time he has his Interview.....having to get the $$$ together.....especially because the Dr. in Morocco didn't want to give him a copy of his shot records :ranting: , we have to get the blood work done to verify that he has the antibodies.....the Civil Surgon we are choosing is not so bad, $150 to do the blood work, give a tetanus shot and fill out the form.

Yup, $150 is a small price to pay to keep USCIS happy and moving along with the application.

Update - I happened to be passing one of the Civil Surgeons listed in the Van Nuys area today so decided to pop in and enquire about the I-693A form. The lady at the front desk was very helpful and knew exactly what I was talking about. She only charged me $20 to get the information transferred from the DS-3025 (which was given to me at the time of my medical in London) to the Supplemental Form. So after lots of phoning around I finally have that part completed and at a very reasonable cost too. Plus she gave me a photocopy of what was in the sealed envelope.

If you happen to live around the Los Angeles area and need to complete the I-693A do give Mari a call, she was the lady who helped me and she deals with all the INS queries.

I didn't submit an I-693a at any time, and I never even saw the DS-3025. I presumed it was in the sealed envelope I submitted at the POE, and nobody ever questioned me about it, although I did almost worry myself into a coma at the AOS interview.

Our civil surgeon was only vaguely familiar with the form transferring and said they've only done it a couple times and didn't know how much to charge so they didn't charge us anything! :P I knew they should have so we were sure to book it out of there fast before they realized their mistake ;)
