Hi Every one,

do I have to send the I-693 form signed by the doctor for AOS? My fiancee came with K-1 Visa, she took her Midical test in Turkey and I only have the Vaccisnation record that Dr. gave us. she took the Med-exam in Sept-2009.

does she have to do the whole process again before I send the AOS packaged?

I called the doctor and he told me that she has to do the whole process again and it cost $250 for X-ray, Blood and vaccination.

should I send the AOS package without the I-693 and just the Copy of Vaccination card?

Were ALL vaccinations complete and are they documented as such on the DS3025?

If yes, you have a chance at sending it with only a copy of that included. Just know that some people have been RFE'd for part 1, 2 (vaccination section only) and part 5 of the I693, so it could result in an RFE.

If no, then she must complete her vaccinations and have them transcribed on to the I693 by a CS. The CS doesn't have to do the vaccinations, just copy over the records. You don't need a new medical or x-ray or blood tests if her medical for the visa was completed within a year of filing. There might still be an RFE.

There are a variety of ways that VJers respond to RFEs for the medical depending on what they had when filing and what the RFE asks for. You should come check things out in the AOS forum as it will have more info for this stage of your process.

I just submitted my wife's AOS package as well and all I included was the vaccination supplement sheet. The box was checked incomplete due to her not having any vaccination records but they gave her the age appropriate vaccinations. I haven't read anywhere about having to do a I-693 if you are a K-1 holder with a valid medical done in the past year. The medical results are sent to the USCIS from the hospital that you had them conducted. So they USCIS *should* have a copy of the medical already. Here's what it says right on the I-485 instructions:

"If you are a K-1 fiance(e) or K-2 dependent who had a medical examination within the past year as required for the non-immigrant fiance(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your form I-485."

It says what it says. All I did was follow their instructions. So why should we get punished with an RFE?

Your argument is a good one, but the problem isn't with you or me, its because they have people who don't know anything about K-1s opening packages & checking everything is there, and the medical is on their list of things required. They don't always seem to make an effort to match up the packet returned at POE with your new filing, so LOTS of people got RFEs regardless of how well they did or didn't follow the instructions.

Here is what I think most people and Civil Surgeons are missing. If you follow the link above to USCIS for the Q + A on TB skin testing and vaccination requirements, you will see the following Q + A.

Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted.

Because the last page of the I-693 (where the doctor signs) says that he certifies that he , the CS, did a med. exam, he may not want to sign it without him doing an exam, thus charging you a fee for another exam. Copy the entire Q+A and bring it with you to convince the CS you will not need another exam regardless of any other immunizations or TB skin test. Good luck! Mari-Joe

I see countless posts about these same issues. Some of the problems are a LOT of CS will not sign off on ANY portion of the I-693 even though you have them read along where its states another exam is NOT needed if the previous exam is less than 1 year ago, in which case it is with my wife. I had a CS lie to my face that she would do the transcription for $125 in a phone conversation, which is way over priced, but when we got to here office, which is on 5 minutes from my home, she refused to do it and wanted $415 for an exam. I just left her office, and of course I was pissed, and drove to another CS that said she would do the transcription for $10 or a full medical for $165, needless to say I drove the 50 miles. It don't matter if you know the rules or NOT, these CS do damn well what they please, and they know they have us in a bind.

The vaccination supplement it's referring to is the vaccination section of the 693 (which is in the list of required items and then reading the 693 instructions explain the "vaccination supplement" part is just part 1, part 2 (vaccination chart) and part 5). Also, if her record says incomplete then there are some shots she needs in the US. if she's complete it would have the "vaccinations complete" ticked.

There is a lot of information regarding the 693 and K1's, especially in these threads. If you have shots done in the US (that's when it says "incomplete) then you must see a CS to have the vaccinations transcribed. If you didn't (when it says complete) then you can safely send just the DS-3025.

also as stated above, there are people who have a checklist that they follow. It's most likely that you will get an RFE UNLESS her incomplete is because of the HPV not being complete because it's not needed for AOS anyway.

If I were you I'd start calling CS's to get a quote so you know what you're looking at. Remember that it's only the vaccination transcription, a new medical is NOT required. It should cost around $50 (hopefully less).
