Hello,

I am 23 years old, so it was demanded that I took the HPV vaccine. It consists of 3 shots: started taking it in December, when my panel physician in Brazil took note of it on the DS-3025, and then in February, a week before coming to US, I took the second shot, so that is not marked on the DS-3025, just on my personal vaccination records. Third shot is due to August 11th.

I have a copy of that DS-3025, she filled two and gave one to me, signed. But there it is marked that my vaccine history is indeed incomplete, the HPV vaccine is marked "insufficient time interval" and below all that it is marked "applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appopriate (as Indicated Above)". Also, I have many others marked as "Not Age Appropriate".

But then, HPV vaccines are not required anymore:

Will the HPV vaccine reccord not matter in this case? Or will they only see that it is marked that my vaccination record is incomplete?

So, should I see another panel physician to make another DS-3025 to update my vaccination record? And if so, will I be required, once I complete the last shot of the HPV vaccine, to mail yet another DS-3025 so that my "vaccination status" becomes clear?

Also, will I need the extra forms as said here?

(From "FAQs: Forms I-693 and DS-3025, Medical and Vaccination Supplements When Adjusting Status") "Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application."

...or is the updated DS-3025 enough when it is just needed to update a vaccine status?

If I need these extra forms, should it all be done by the doctor updating my vaccination record or should I try and complete it by myself first?

Also, how can the doctor tell if my exam done prior to coming to US is not older then a year, so that I don't need to do it all over again? Can he doubt that and ask for evidence? The only document I have about it is this DS-3025, which is properly dated.

When ready to process your AOS, you will need to see an approved physician. He/She will complete for your the I-693 with all the necessary vaccination information required by the USCIS. If you are missing any require vaccination, the physician will advise you and provide the service.

Yep, when you're preparing the papers for the AOS, you'll take your vaccination card and the copy of the DS-3025 and ask the approved physician to update the information for you, he should then fill out a new form with the completed vaccination sheet.

I left Brazil with 3 shots incomplete (reforços) which I got done here after I arrived. I had no problems getting the information updated for my AOS.
