My wife came to the US with a K-3 Visa she had her medical examination prior to entering the US. She has her DS-3025 but is blank the doctor did not mark anything on it he just signed it. Will this be accepted? What should we do?

If it is blank then it it incomplete, if it is incomplete then it won't work

Visit a civil surgeon

Won't work. Did she receive any vaccinations from the panel physician? If so, get those records (and any other records of immunizations) and go see a civil surgeon.

The DS-3025 is just the vaccination work sheet. My fiancee only had one vaccination (the minimum allowed so she could enter the US). If she has no record of vaccinations she will need to get them again unless she can her prior records. You dont have to use a civil surgeon to get the vaccinations. My finacee just went to the local Health Department yesterday and got four (and they don't charge anything). In a few weeks she will get a few more. We will then take the vaccination record with us when we visit the civil surgeon to get the appropriate sections of the I-693 completed. (after our upcoming wedding!)
