The documents I need to submit are here with me now. Now, I have to fill up the forms.

I do have a 4 year old daughter. Do I need to fill up a form for her even shes 4 years old? If ever, what if I dont know some informations about her father like, the place of birth and current address?

Each intended visa holder needs an application... so yes you need to do one for your daughter. Answer the questions to the best of your ability... Do you have permission from the father to allow the child to emigrate?

Oh, thank you so much. I dont need the consent of the father because I have the full custody of my father.

Not sure how it works in Thailand but in some countries "full" custody is not enough

Yes, shes using her fathers family name but I am from Philippines and theres a law in Philippines that says, the mother has the full custody... that was my first problem i ask helped from here and so far with same cases with me from Philippines, they said, i dont have to worry as long as we are not married.

Hi Ann, I hope you are doing well. I would like to see if you can get some more help from your USC fiance.

If the father's name is on the birth certificate you will need to try to locate him and show you could not. On the forms for father's address, write "Unknown" or something similar. "Whereabouts unknown."

But if his name is on your daughter's birth certificate, you will either need to get a legal document permitting you to take your daughter out of Thailand to the USA or show you have tried to locate the father but cannot. SInce you are in Thailand but from the Philippines, I am not certain how the US Embassy in Bangkok will handle the Visa application of your daughter.

Good luck. Ask more questions if you need to.

Please don't confuse the OP with incorrect information. She's from the Philippines where the father has no parental rights unless married to child's mother. She already knows this.

But she is in Thailand. She will be interviewing by the rules and policy of the US Embassy in Bangkok Thailand which has authority over this matter. Not the Philippines.

It doesn't work that way. These are public threads. A Filipina obtaining a visa through the IV unit in Bangkok doesn't need permission from a child's father. What makes you think otherwise? A Thai fiancee is in a different boat.

Think it through. If she and the child are in Thailand, would she need permission to take the child out of Thailand to return to the PI? No. The same applies to leaving Thailand for the USA. The Consulate will apply the correct regulations for a Philippine child of an unmarried mother as they would if she were obtaining the visa in Manila.
