Long Term Goal #1 Joshua will improve his adjustment/stress management skills and implement learned coping skills into daily living.

Joshua is a 18 y/o Filipino male readmitted on approximately October 12, 2009 after complications due to a replacement bone flap surgery on September 10, 2009. Joshua originally sustained a traumatic brain injury secondary to a bicycle accident on June 17, 2009. Rehabilitation Counselor/social worker conducted a psychosocial assessment on October 26, 2009. Joshua was fully oriented. Although his speech was slurred at times and he presented with noticeable processing delays and wording finding challenges, he was able to communicate his needs. Joshua was easy to interview, as he was cooperative and answered questions appropriately. He reported problems with concentrating. Joshua reported and no observation of hallucinations/delusion. He reported and observed were memory deficits and word finding challenges. Joshua was ambulating with a quad cane, and had limited movement in his ® arm/hand/fingers. Joshua was given a self-report self-esteem questionnaire and scored relatively high (90/100). However, he expressed self-esteem concerns. In addition, he reported feelings of sadness, being easily frustrated, and having less patience; yet, he was hopeful.

• Joshua has been an active participant in individual and group sessions where he was always prepared to learn. Joshua passionately explored his thoughts, feelings, and concerns in individual sessions.

He has modeled appropriate behavior and demonstrated leadership qualities in the teen group sessions.

• Joshua has learned of positive affirmations and engaged in self-esteem boosting assignments throughout his rehabilitation. He also engaged in numerous assignments/activities that elicited self-awareness and self-talk, foster understanding and acceptance, and gave him the comfort to express feelings, ideas, and beliefs. He has started college while at NCEP, and has been able to implement the knowledge learned in counseling sessions into his college course. In his college course, he was given an assignment of developing an affirmation. According to Joshua, he was excited and completed the assignment with ease, as other students were unfamiliar with the term.

• Joshua reports that his self-confidence has improved. His feelings of sadness has reduced from less to none with medication management and applying the coping skills learned in sessions. Joshua was given the identical self-esteem questionnaire on February 5, 2010 and scored 95/100. His self-esteem appeared to improve as he began to recognize progress in all areas of his rehabilitation.

• Joshua initially presented shy; however, he became comfortable and displayed his funny sense of humor. Joshua’s initiation increased as he began to volunteer in out loud readings.

• Joshua learned of stress management, anger management, and coping skills. He engaged in deep breathing and visualization imagery. He learned the stages of anger and verbalized his emotional and behavioral responses during each stage. Joshua identified his stressors, as well as his physical, emotional, and behavioral responses to stress. Joshua was instrumental in helping the group develop a list of coping mechanism in order to manage stress and anger, such as using humor and self-talk.

• Joshua journal his thoughts and feelings at home by computer. He also composed his emotions into lyrics. In addition, he worked with a staff and developed a meditation CD to be utilized in NCEP group sessions under the guidance of the vocational specialist and occupational specialist.

• Joshua engaged in problem solving to made-up and real scenarios and verbalized appropriate problem solving solutions throughout rehabilitation.

• Joshua completed all homework assignments, as he kept track of his emotions (identified the situation, thoughts about the situation, and emotion at that time).

• Joshua learned the stages of grief and explored his feeling/experiences through each stage.

• Joshua developed a self-care/commit to change plan with goals, action plans, and target dates. One of his goals was to start college.

• Joshua attended several bowling outings, and an experience at the Lied Discovery museum.

• As his self-esteem increased, Joshua has learned how to reframe his disappointments and/or experiences as opportunities. He’s interested in becoming a motivational speaker addressing helmet safety with elementary and middle school students.

Some people are not going to like this but IMO

Ok maybe you should explained to him. i dont want to live some where.. where life as I am used to.. is not always secure. The options America leaves for people are unbelievable. Some Jamaicans dont agree with the culture of American and thats fine, you can have Jamaican culture in America. Some people are not used to getting everything the american way.. and dont wish to bend to anything else. One thing about the US, you can do any rass thing you set your mind on in this country, and actually make money off it. And i love that.. in jamaica. the smallest idea can take you some place.. but it will never take you to far.. unless you hussle your way. The jamaica his parents grew up in is gone. For good... the jamaica he probably grew up in is getting worse, not better, and the main reason is= all the educated are leaving (SMH), and not coming back to make it better, so if he want to go back he should stay and make that money and maybe one day he will realize he love his country enough to try and return to help it out. Not go back because its easier. But one thing is and this is so true Americans are lazy and a Jamaican can have alot here because of their work ethic.

i am thankful that we all got to be with my dad his last mnth.. we talked and laughed and i know it was God cuz he could have easily died without us having this chance..

question...what do you think about cremation.. i personally have nothing against it, but some familymembers are tripping for real.. my dad has no money and cremation is the most affordable route but for a couple more hundred we could have just a gravesite burial w/ no head stone..

just looking for a lil more knowledge on the subject

Glad you seem to be doing okay despite the circumstances Q!

I'm with the others, my preference is cremation. To me it is creepy to be laying in the ground rotting.

Once again Q sorry for your loss.

As far as cremation I am all for it. That is what I have requested for me when I pass. I don't want to be in the ground somewhere. Everyone has their opinion but if it is the cheapest way to go then that is what I would do. As they say ashes to ashes, dust to dust.

You said it for me so I didn't have to.

I am pre-paid for cremation.

It's a personal choice. I saw "Poltergeist".....that's pretty much all I am saying on it in public in case I offend anyone.
