Unfortunately, IP addresses are not a good unique identifier for a variety of reasons. There really isn't a good automated duplicate account detection algorithm that I am aware of.

I have a fairly good one that detects about 80% of our duplicate accounts. As an added bonus, it also won't let anyone with an account register.

ETA: I can tell Ewok how to write a basic duplicate account ID, if he's interested, but it will probably have a decent false positive rate given that there are plenty of couples who both post here. That could be mitigated by an addition field in the profile that lets you link your account to your spouse. I think it could probably be adapted to edit a non-editable custom field for mods/admins to see in the profile that links all the accounts attached the the account.

No. IP-based anything is silly because of proxies and dialup and open networks. You'll get far too many false positives strictly using IP.

ActiveX and VBScript get you much further.
