I am not sure if I need the I-693.

When I did my medical examination, I got all the vaccines and the doctor game a sealed envelope saying that I would need that for adjustment of status.

The envelope has a stamp that says:

"DO NOT OPEN THIS ENVELOPE - It contains a copy or your medical examination.

Please provide it to USCIS when requested in conjunction with your adjustment of Status"

Do I still need to submit the I-693.

In order to plan in advance I called the Doctor to make an appointment for the day after I arrived, and They asked 200$ just to fill it out, and if I need vaccines 350$. Is this normal? I was reading the master tread and it says that it cost like 50 to 60$.

Please advise.

Do I still need to submit the I-693. The sealed envelope that you described above SEEMS like it is I-693, Medical Examination. I cannot say for certain, though. In my case, I printed the I-693 forms and had them filled out for my CS. So I know what are the contents of the sealed envelope.

In order to plan in advance I called the Doctor to make an appointment for the day after I arrived, and They asked 200$ just to fill it out, and if I need vaccines 350$. Is this normal? I was reading the master tread and it says that it cost like 50 to 60$. $200 is ridiculous, IMHO. I paid $30 (my insurance covered $45). In my case, one vaccine ($60 from Safeway) was required. I would call other CSs in your area. I think the price is way too high.

Please advise.

By the amount you stated, it seems like you are being charged the price of a full medical. Now I truly understand why people on VJ are so outraged. These Civil Surgeons, some of them, are just ignoring the instructions if the medical has been less than one year under certain visa's, then another medical is NOT required. I wish these Civil Surgeons were monitoried by USCIS, not just appointed or whatever. The one close to me, wants us to pay $415 for a full medical, but over the phone she said she would do the transcription only of the vaccinations over the phone days before we went to see her if thats all we need. Classic bait and switch one I got to her office. I will be driving 50 miles to another Civil Surgeon that will do a full medical for $165 and she even said if I wanted just the transcription of the Vaccinations, she would charge $10.00. I am LIVID to say the least at the one near me. Yes, you still need to submit a I-693 by all means.

$200?!! Hells no - we got charged just $20 to have our medical transcribed, $30 for a blood titre to test for vaccine, and all without insurance (I actually had it, they just wouldn't take it).

I'm so sick of these sham CSs and their inflated costs and their "you need to have another medical" nonsense. We drove to Santa Ana for our CS after all the ones in our area refused anything short of a new medical. Argh.

As for the first question, it sounds like they transcribed the results to an I-693 straight away at the medical. If that's the case, then lucky you! If it's a brown envelope, then that's the standard do-not-open envelope meant for POE. Maybe describe the envelope? Our CS gave us a sealed white business-sized envelope for our I-693, stamped with "do not open," so that's why I ask. :)

I have attached the scan of the envelope. It has the same sticker on the other side.

Do as the envelope says (send it in), and send in a copy of whatever else you have (you should have DS3025 accessible to you).

Sounds like at least some consulates are doing their visa applicants a favor for a change! Go Italy! :)

I actually had the interview in Frankfurt. so honor to them.

I do not have a copy of the DS3025. So maybe that is what is inside the envelope. I only have the brown envelope, the white envelope, and the cd rom for the x-rays.

I will call the doctor and the embassy tomorrow to ask anyway.

I know this post from you is old but can you tell me the Doc you used in Santa Ana please? So far the cheapest I found was still $130 with flue shot included.
