So i finally picked up my results from fedex.

i got this paper attached to the sealed do no open envelope, that says worksheet two.

where it says my td tdap shot, the column for "insufficient time interval" box is checked. is that because of the lenght of time between my shots? the last one i got in my vaccination book was 89, and its' suposed to be every 10 years.. so i had it done in january...i'm just wondering what that box actually means.

also it says at the bottom application may be eligible for blanket waiver as indicated above..but i dont see anything indicated above??

everything else says either not age appropriate or in case of my mmr it says copleted.

they said everthing was ok..but i'm not sure? lol. also is this the sheet i need to have transcribed to the i-693?
