I-134 Supporting evidence questions

My mom is going to be a co-sponsor for me, what should she put as the relationship to sponsor?

Also, what forms do we need in conjunction with the I-134 (for myself and co-sponsor)?

Did I need the G-325 for my co-sponsor? If so, then what do I put in for "This form is submitted in connection with an application for:"?

Thanks for anyone who can help!

"Mother of petitioner" or, "Future Mother In-Law" (or something similar).

Also, what forms do we need in conjunction with the I-134 (for myself and co-sponsor)?

The only form you'll each need is the I-134 aka Affidavit of Support. You will both need to submit the form and supporting evidence/documents (this may vary, depending on how you both earn your income).

Did I need the G-325 for my co-sponsor? If so, then what do I put in for "This form is submitted in connection with an application for:"?

No G-325A for mom and "K-1 Visa" (perhaps the Example I-134 would help.

** Topic is Interview/Embassy Related; moving thread to "US Embassy/Consulate Discussion" where these types of questions may be more appropriate.

James' mother is our co sponsor. We wrote "Mother in Law to Be", and attached a copy of her birth certificate and driver's license (a passport is the best document, but she does not have one). Both I-134, James' and hers were accepted without a question.
