Guessing that you will be going to the London embassy:

Ask your physician which ones he gave you in the past. At least here you can do that. Other than that, you should have and will for the medical exam require your shot records!

Shirorisu -- as long as you are under 50, the only jabs you will need are the Tdap and the MMR. Since the varicella vaccine is not available at in the UK, as long as you have had chicken pox, your verbal assertion will suffice for this.

Moving to the UK sub-forum as this is not just an IR-1/CR-1 issue, and is UK-specific. :)
