my father is dead and his name is not included on my birth certificate and he died when i was young so i dont remember his information do i include his name on the form or i can use my step father

You can not use your step-father on the G-325A. IS it possible for you to ask your Mother to provide your Father's name and birthdate?

If you can not find out information regarding your father, list "unknown" in the name area.

If you know your father's name, list it. IF you do not know his birthdate unknown is acceptable.
