AOS submission- Immunization?

I'm ready to send in our AOS package but just realized we did not have DS-3025 or I-693. I did not see it as required in VJ AOS guide line.

Can some one shed some light into this matter? My wife got her medical exam in VN last December. Does she needs any other medical exam or paper work to submit with our AOS package.

Also below is our package list. Are we missing anything. Thanks million for your help..

--AOS I-485 Form--

- G-1145 – Form for E-notification of Application/Petition Acceptance

- Payment in the amount of $1,010.00 payable to U.S. Department of Homeland Security

- I-485

- Copy of birth certificate

- Copy of K1 visa in passport

- 2 passport photos (US passport size)

- G-325A

- I-864 - Affidavit of Support (Petitioner)

o Copy tax return, including W-2, 2008 tax year

o Copy tax return, including W-2, 2007 tax year

o Copy tax return, including W-2, 2006 tax year

o Letter from employer

- I-864 - Affidavit of Support Under (Co-Sponsor, in case petitioner not qualify)

- I-765 - Application for Employment Authorization

- 2 passport photos (US passport size)

- Photocopy of passport biographical details page

- Copy of I-94 – Arrival/Departure record (front & back)

- Copy of Marriage certificate

--AP I-131 Form--

- I-131 - Application for Travel Document (Advanced Parole)

Okay, well it looks like you copy and pasted directly from my AOS thread and you can see from that thread that it DOES mention the DS-3025 under the 2 passport photos. i also discussed whether it needs to be included and copied some threads for reading.

She didn't get the vaccination supplement sheet from the doctor? Did she go to Cho Ray? You will need this or another medical report.

I just sent in my AOS package recently... and I sent in both DS-3025 and I-693. I think I-693 is essential if your DS-3025 is incomplete as it was in my case... and also, you can always call back to your approved doctor back in your country to ask for the DS-3025. I have seen a lot of cases here getting RFEs because of insufficient information on DS-3025.... so it is important to have sufficient info for medical forms, be it DS-3025 or I-693.

We only needed one G1145 for the whole package....

If her vaccinations were complete on the DS3025, then you can try to send that in. Don't forget the flu shots are also required during the winter time. If not, or if you just want to play it safe (especially if you can find a CS for only $30), have it transcribed to the I693.

K-1:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

To be clear; my fiance DS 3025 only have Td checked and on the bottom marked " Vacine history incomplete". This means I need to contact a local CS to complete the I693, correct? :(

They should have told her what vacinnations are still needed, you will need to get those done and then have the CS transcribe the vacinnation record to the I-693 (including the completed vacinnations that were previously missing.

It will probably be best for your pocketbook if she completes the missng vaccinations with her regular doctor or at thepublic health department and brings the documentation with her to be transcribed.
