Do we both fill out the form, or only the beneficiary does? Also can he appoint me as an agent to receive the paperwork?

Also do I have to submit original signature, or is photocopy sufficient?

The beneficiary must sign it, to designate the petitioner as 'Agent'.

It must be filled out, doesn't matter WHO fills it out.

It does matter WHO signs it, though.

Thank you, can the form be emailed, or do they need the original? I the guide I read, it stated that the beneficiary needs to sign and mail the form, but I see a lot of people are emailing the form to NVC. Does it mean they do not require originals?

there is a form letter on the shortcuts. You can do that and send it from the benificiary's e-mail address. That is what we did and they accepted it like yesterday or the day before. Check my signature for when i sent it and then got my IV bill invoiced and paid. (they need the choice of agent to invoice the IV bill)

You can just email it in letter format. Takes about 7 days or so to be accepted.
