I-693A

I am finishing off the paperwork for the AOS. I was able to get my I-693A without having to actually go the the civil surgeons office. I just sent them a copy of my vaccination worksheet and a money order. I received a sealed envelope with the original vaccination supplement in it and a photocopy of what was in the sealed envelope. Nowhere on any instruction sheet does it say that you can't open that envelope. I was reading some of the posts and some say to send the sealed envelope. Does it really have to be sealed? Please, oh please say NO.

I'm sorry to say this, but you shouldn't open it. I has to go like that, sealed. If you already open it, I believe you should ask the doctor what to do, maybe you have to do the same process again. You just have to keep the copy with you and send the sealed envelope with your applications.

Just wait for more replies.

I just did mine last week and was told not to open the sealed envelope, because we need to send it with our AOS application.

I was told that the envelope had to remain sealed. If you opened yours, you might try to talk to the doctor to see if they can seal it in a new envelope for you.

I agree, the envelope HAS to be sealed......call the doctor, send another money order and have them mail you a new sealed envelope

Thanks for your answers. Not really what I wanted to hear. And to think, this wasn't one of the things I was concerned about. I'm sure glad I read other posts. I'll call the doctors secretary tomorrow and see if I can just mail back the I-693A and have her put it in another sealed envelope. She was very nice, so she may just do it for me

We sent ours in the sealed envelope (although I did write on the envelope saying that the passport number on the form was from his old passport and we also punched holes at the top to attach it to the ACCO fastener....hope thats ok!).

i didn't open mine because she licked the envelope, stamped it like 15 times all over the flap and taped it down like it was the colonel's secret recipe in there. and she looked me dead in the eye and said "DO NOT OPEN IT!" they gave me a copy for my records so i had to reason to peek.

maybe you should just send them a self-addressed stamped envelope with the I693A so she only has to seal and stamp it and send it back to you. good luck!

That's what I did. Called her, sent her back the I-695A that was in the envelope with a return envelope. Sent it on Monday and got it back on Wednesday
