Amendment of passport

I've been trying to call the Philippine Consulate but it always goes to voicemail for a week now, so not lucky at all. I had my passport name under my previous annulled marriage in the Philippines and I plan on having it amended to reflect my current married name. I understand that I have to do the report of marriage with copies of marriage certificates and money order for the charge. What other requirements do I have to send? NSO marriage certificate with annotation? or the certificate of finality? For those that have already been in this road please help!!!

Hi...right now we are doing the same thing....we looked on the internet, saw that the phil consulate was doing and "out reach program" close to our area, so we just send in by fax the required forms and documents, now we have to go in person to give money, fingers prints and passport type photos. You have to complete a Report of Marriage form which requires, copies birth certificates of both, copies of passport and ID/Drive License, and divorce papers of husband. they have a list on the website of required documents. Also they are going to a "readable" passport and suggest that any person who currently has a "green" passport renew and obtain a readable passport. You have to appear in person to get a renewed passport because of finger prints etc.

We live in the San Diego area so they are coming down on a saturday, which makes it a lot easier for people to get the new passports, without having to go to Los Angeles during a weekday. Check out the consulate website that you come under and you might see what I am referring to, good luck.

First of all, find the Philippine embassy/consulate that has jurisdiction over your state here: http://www.philippineembassy-usa.org/Consulates.htm

Then, google the website for your consulate and check what their requirements are. Our embassy/consulates have different sets of requirements.

Thanks Pinaywife & mark&rhiz for the quick reply! I have the basics like the forms that I will have to fill up and the MC, its not explicitly mentioned in the Chicago consulate if I need the certificate of finality or the annotated MC, really hoping for someone who has been through this already to give inputs. I guess I will have to wait after mark&rhiz have done their's... thanks again!

Hi cjthel.

I renewed my Philippine passport at DFA Manila.

I brought the original Annulment Papers with the Certificate of Finality attached at the front (prepare a photocopy of this for their filing purposes), I also provided my NSO Copy of the Annotated Marriage Certificate (which they took and didn't return to me). You also have to show them your NSO birth certificate and your new Marriage Certificate to show how your name legally transitioned from your single name to previous married name to current married name.

02-23-2018: Date on Biometrics Appointment Letter (Biometrics Appointment at Jacksonville ASC on March 13, 10:00 a.m.)

03-08-2018: Biometrics complete

04-05-2018: Case status updated - Interview Scheduled on May 10, 2018, 10:15 a.m. :D

05-10-2018: Citizenship Interview - Passed English and Civics Tests, Recommended for Approval! :D 

06-19-2018: Received email and text notification: Naturalization Ceremony Scheduled; waited for letter to be uploaded on online account - it has been set on Wednesday, July 25, 3:00 p.m.

07-25-2018: I am now a U.S. Citizen!

Ouch! I only have one copy of the annotated NSO marriage cert. Thanks so much Marriedtomrg!!! This info is what I need.
