Timeline

seems the physical papers were kept at POE when my wife got here. So I am going to have to get new physical done when calling her doctor here what all needs to be filled out? Is is just a regular physical?

Do you have a copy of the shots? That is all that needs to be resubmitted if she has had the physical within that past year. Read the I485 instructions and the Physical paperwork instructions very carefully. I had to highlight and explain to my civil surgeon but finally got through to them.

Do you have a copy of teh DS-3025 Vaccination Summary (obtained during the medical done before the interview or copy obtained at the interview)?

If so, then you submit this AND (as suggestd by the Guides here on VJ) a partially completed I-693. For the I-693 the Civil Surgeon only needs to transcribed the vaccination records from the DS-3025 onto the I-693 and seal everything in an envelope. YOu then send in a copy of the DS-3025 and this sealed envelope (regardless of what the I-485 instructions lead you to believe, that is, the I-693 is not required).

If you do not have the DS-3025 and if you have no other documents from their country that can tell the Civil Surgeon what vaccinations the person has had and when, then you have to redo all the vaccinations (the "physical" as you call it) again and the CS will fill out the I-693 completely.

Just to point something out - according to your timeline (US entry, end of March 2009) I'm guessing her medical was shortly before that; this could make her original medical no longer valid (they're only good for 1 year).

If that is the case, she may need to take a new, complete, medical exam by a qualified civil surgeon (it may not be as 'simple' as getting the vaccinations transcribed). Check your documentation against a calendar to be sure.

I have found the vacatioin worksheet with all vacacinations listed. I am still a little confused where is the form 693 I dont see it listed on down loads.

Just to point something out - according to your timeline (US entry, end of March 2009) I'm guessing her medical was shortly before that; this could make her original medical no longer valid (they're only good for 1 year).

If that is the case, she may need to take a new, complete, medical exam by a qualified civil surgeon (it may not be as 'simple' as getting the vaccinations transcribed). Check your documentation against a calendar to be sure.

Thats the scary part I know it was done in Feburary but not sure of date, as far as getting transcribed I have nothing to transcrie as all papers were kept at POE

Look at the Example Form. The I-765 is a multi-use form. For K-1s, this section is left blank. That's what I did and no problems. Leave it blank.

If you don't have a copy of the DS-3025 form or any official other vaccination summary form, then there is nothing for the CS to transcribe and you have no other choice but to repeat the examination and the vaccinations again.

Okay found the date she got medical was Feb. 25 now is this going to be enough time if I send everthing in this weekend? It was good for a year I know but I dont want this sent back

I found the DS 3025 form and have it here now I am still wondering about the form do I take the 693 to surgeoun or just attatch this form to it and send it in with paper work I know I might sound a little slow but I am just being thorough should have had this done but its been busy year with new daughter and hard pregnacny.

You're cutting it pretty fine. If you think you can get it sent off this week (Probably want to overnight) so that your NOA1 is dated at the beginning of next week you'll just barely file before it expires. It's the filing date vs. the medical date which matters.

Then, let them RFE you for the transcription of the DS3025.

They may also say that the medical is out of date, in which case you loose the gamble and you just go get the full medical.

If you want it all completely out of your hair, then do the full medical and bring the DS3025 for them to check her records and put on there. Full medicals aren't that bad - I've seen prices around $150. Sounds to me that right now what you need most is to just have it done right the first time instead of the runaround.

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.
