Hi I posted this question last week and I am still a little confused. I am sending in all forms ie. work, residency the whole works. I have a few questions and have broke them out for some HELP PLEASE.

thanks in advance.

What all forms need notorized?

The birth certificate is not translated into english, how do I A] get it translated and B] have her mother sign it

I know there are forms on the internet that I can print this out but on her original her mother has signed it.

Also I am sending in my w2 form for this year will this be sufficent.

Can get it either professionally translated or have someone fluent in both languages certify the translation. You'll need the original version along with the English translation.

I know there are forms on the internet that I can print this out but on her original her mother has signed it.

I'm not sure what the mother has to do with the birth certificate (besides the obvious).

Also I am sending in my w2 form for this year will this be sufficent.

Thanks for reply I see that I have to send in completed taxes also with w2 also, on the section for application type im wondering if I should check for advance or the box for the one that says conditonal resident If I mark the advance parol I have to send in letter stating why she wants to go home and I am thinking to show our new born child really does not quallify....does it

You're referring to the I-131 Travel Document/AP - most folks will check "D" Advance Parole (as seen in the example form). You really don't need a reason unless you're applying for emergency purposes. They have no reason to deny AP.

Once again thanks so the total I will be paying will be 1010.00 correct and also another problem has arisen seems that the physical paper work was kept in Ft lauderdale. How do I go forward with this only thing I can think would be getting another physical. This is turning into a nightmare.

Yes the total fees paid would be $1010 ($930 and $80 biometrics); this would include (because you're sending them all together - which is good) the AOS/I-485, EAD/I-765 and AP/I-131 (the I-485 you have to do, the others are optional).

Medical is a whole other subject. Judging from your timeline, the medical took place somewhere in Feb. 2009? which may make it more than 1-year old (use your calendar to be certain) in which case a new medical may actually be required.
