Name on I-693 - maiden or married name

Hi,

I just got married and my marriage certificate only lists my maiden name. When I go for my medical to get the I-693 form filled, which name will the civil surgeon put on it? Will it be a problem if he puts my maiden name on it, since I'm filing my AOS forms in my married name, as I want my GC to come in my married name.

You should put your married name so it's consistent with the rest of the AOS paperwork.

Keep it simple. if all other forms are filled out in your married name, use it for the medical as well.

My civil surgeon didn't really look at my ID. I just handed them my forms filled out with my married name, and he did the rest accordingly.

Which forms did you take to the civil surgeon? Just the one part of I-693 filled out?

you don't have to worry about it. i went to my civil surgeon for I-693 under my married name, i even have my marriage cert with me and everything... they don't even look at it. the best evidence if they ever ask for it is your marriage cert.

I actually went to the civil surgeon for my exam and filled out the paperwork with my married name even though my passport and all my other ID's are still under my maiden name. The surgeon actually took a copy of my passport which he wants to send out with the I-693, what should I do now? Is this going to be problem?
