I-134 Questions

Hey everyone, I'm filing my K1 for my fiance, but I need a little help filling out this form. My father is cosponsoring my fiance and I have a few questions:

1. How many I-134's should be included? I'm guessing one for me and one for my father? Or just my father?

I am unemployed and did not file 2009 tax returns. How many previous years should I include? I did file 2008 and 2007, should I include these? Or just leave everything to my father's returns?

The US Citizen fiancé/spouse is the primary sponsor regardless of income (so that's one) - if using a co-sponsor, they'll need one also (so that's two).

You (and your co-sponsor) will need to submit at least the most recent year's return (additional ones if you feel it will help) so you decide.

I have another question. My father who is co-sponsoring me was born outside the US, was naturalized about 30 years ago, became a US citizen legally through residency and US citizen courses about 15 years ago. How do I answer Question #1 of the I-134? His residency card was surrendered when he obtained his citizenship, and the only legal proof is his "Citizenship Diploma" he received, and current passport. Should I choose option C and attached copies of his passport and citizenship certificate?
