Ok on the Riyadh website it says

For cases filed in Saudi Arabia: The U.S. Embassy in Riyadh provides all instructions to the beneficiary and sends the appointment letter.

Once the beneficiary has received the appointment letter--and no sooner--he/she may proceed with the medical exam.

Appointments are scheduled approximately 5–6 weeks in advance.

My fiance sent the DS230 on Monday and he received the interview letter today. It was just one page. My questoin is he took his medical examination before he got the interview letter because when he received the packet 3 it gave him the instructions on the Medical examination. My question is he hasnt opened the sealed envelope, but it will probably be dated before they sent out the interview letter. Is that going to cause any problems or we will be givn a 221g or something? I want to make sure things are done correctly so any advice, please please please help!

As long as he didn't open the envelope he received from the medical exam then he is fine. The date of the results is not important because you typically get the medical exam done after receiving packet 3. The fact the interview date is after the date medical isn't important as you don't get that letter until after you send back the DS230. He just needs to take the "sealed" envelope with him along with the other required documents and he will be fine. Make sure he takes a lot of pictures/ emails and chat logs with him on the date of his interview. That one page letter he received that notified him the date of interview, he will need to take as well because they will ask for it. My fiancee is Bangladeshi but his interview took place at the Riyadh embassy because he works in Saudi Arabia.
