Hi everyone,

Whoever had their vaccination supplement transcribed onto I693, for AOS after K1 entry, did you need to bring your marriage license with you? I'm thinking that I693 has to be in the married name, not maiden name if everything else is? I haven't got my marriage license certified yet, so should I wait until I have that in hand before I can make appt with the doctor? Thank you

Nope. I didn't need to show them my marriage license. And I applied on my married name. Passport - yes.

I-693 and marriage cert is irrelevant, you are signing the I-693 to promise to tell the truth so the doctor does not need the marriage cert.
