hi:

im filling my adjust of status and kind of confused how to fill the space where you put your name, i have first name, midle name, last name and surname, so i want to keep my last name and also my busband's, my full name before married is CLAUDIA MARIA PERZ ROSALES and my husband's name is STEVE BOYER, so i want my new name as CLAUDIA PEREZ BOYER, how should i fill the spaces, can i write as PEREZ as my middle name or should i leave it blank and in the space of last name write PEREZ BOYER for example:

first name=CLAUDIA

middle name= PEREZ

last name= BOYER

OR

first name= CLAUDIA

last name=PEREZ BOYER

thank you very much for the help you can provide me, is very importan for me to keep my last name and i dont want to mess up with the forms.

It depends. Surname and last name are the same thing. There is such a thing as two surnames (or last names) but that's if you want it that way. For eternity you would always be "Mrs Perez Boyer". It's become quite common for married women to keep their maiden name and turn it into a middle name. So you will be "Mrs Boyer" and Perez will now become your "middle name.

Personally I would prefer having perez as the middle name, but if it's important to you then it's almost like having it hyphenated but it not being hyphenated.

Also, did you write your preference on your marriage licence? Some can, some can't. I could write my "new name" on my licence and i did. In my state if I didn't write it on the licence then I didn't change my name.

well i dint know about changin the name in my marriage license besides the lady in the courthouse never asked me if i want it to change my name so she wrote my name as in the passport.

i m also concerned that if i convert my lastname to middle name, since the middle name sometimes people just put it w and initial, cause i want it the whole names, no initials.
