Hi all,

I came to US 3.5 weeks ago on a K1 visa. My fiance and I got married and now we're getting ready to do AOS. I've been reading up on everything on this site about it and I'm very confused about the medical. I know that I don't have to do a medical because I've done it back in Canada in September. From my medical then, I got a sheet called Vaccination documentation medical worksheet two. It doesn't say anywhere on it that it's DS 2053. I don't have anything else. I'm not sure what to do with this. It does say all the vaccinations I've received (all required ones). It's signed by the panel physician. Then I've read that it should be transcribed to I693 but it's not required for K1, only DS 2053. But I don't know if my vaccination sheet is DS 2053. Thanks for your help.

I had the exact same papers you do. I had it transcribed onto an I-693 by a local civil surgeon. He only charged me about $25US, but did require me to go get Hep A and Hep B vaccinations that the Canadian civil surgeon had marked off on the sheet as "Age Inappropriate" His office sealed the I-693 (which was only 3 pages instead of the usual 6 - That's what they mean by you only having to include the vaccination supplement.) in an envelope, which I included in the AOS packet. Which got approved in 2 months with no RFEs or interviews.

You'll want to search the USCIS website to get the listing of civil surgeons in your area, and keep phoning until you find one that will do the transcription for a reasonable price! There's also a thread here in the AOS forum that is trying to make and maintain a list of CS's whom you don't have to wrestle with too much to get the transcription done. Too many of them will try to sell you a full reexamination. You can try showing them the instructions for the I-693 from the USCIS website, where it pretty clearly states, as you said, that you don't need another whole examination, but too many doctors, it seems, lose the ability to read after they graduate med school. The ability to read and keep up with professional literature gets replaced with the ability to get convinced to issue expensive, pointless prescription drugs by suspiciously persuasive [not to mention very pretty/handsome] young drug company reps. But that's a whole 'nother rant :)

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

Thank you guys! I looked at that list and there is no doctors listed for Michigan, where I am. I hope that doesn't mean the doctor's here are going to make me do another exam!

Not necessarily, it just means that you're going to have to do the phoning and inquiries yourself. The USCIS website will give you (in all likelihood) and big long list of civil surgeons in Michigan. Just work your way down the list, starting with the closest ones to you, asking about getting your vaccination info transcribed on to an I-693 for your Adjustment of Status application. They should know what you're talking about, anyways. And at least one of the CS's on the list should be reasonable. You just have to find out which one. :(
