Sorry, was away cooking dinner for my aunt who is gimp at the moment.

It really is a mixture of both. That pain specialist is the only one in the greater Vancouver area. Why is this? $$$$$$$ Plain and simple. Doctors in Canada start out at around $80k, when they can just go south and double that as a GP and have way more control over things such as their office hours, patient load, etc.
