DS-156: #39 Was this application prepared by another person on your behalf?

Hey everyone-

Sorry if this question is a repeat, but I can't find my specific answer. Have any of you (US citizens) filled out and signed the DS-156 form for your fiances? I just noticed that there are two options for signatures- for the petitioner or the beneficiary. It would be a lot more convenient if I could fill it out for my fiance and sign it myself, for various reasons. There wouldn't be any suspiciousness surrounding this, right? Or is this really only for third parties, like attorneys?

Option 2, can I mail my fiance a completely blank DS-156, have him sign it, and then when he mails it back to me, fill it in by hand? Is the fact that it's electronic only for neatness and filling out purposes, or is this info saved in a database somewhere and pulled up at a later date? Or I could fill out part of the form electronically, mail it to him, have him sign it, mail it back, and then I can fill in the missing details by hand.

Please let me know what you all think the most acceptable option is!

I filled it out on the computer, printed it out and then mailed it to him for his signature. I filled out all of his forms and then mailed them to him, so he could send to the consulate with everything else that has to be sent with Packet # 3.

I didn't sign saying I had prepared the form.I suppose it wouldn't hurt, but I had been told that was only necessary for lawyers, etc.

When you fill out the form, and click submit, it is going to generate a third page that will have a bar code. You will need that barcode to get in to the embassy, so you need to complete the form and print it out. None of the information is stored.. but you need the bar code.

The other forms you could print out and fill in by hand, but the 156 is going to have to be completed on the computer.

Since he will need to mail other documents in with packet three, why not just complete everything for him and mail them to him. Then he can sign and include them with the other information.

The beneficiary should sign the forms. If you're the fiancé/spouse helping the other fill out forms, I really don't think you have to sign here (I'd bet most of us didn't).

The DS-156 (and some others) is something the beneficiary either sends-to or takes-to the consulate so, their completed forms can be emailed to the beneficiary to print out and sign; just a thought.

Thanks for the responses! I know that I still have a bunch of time before these forms will need to be sent in, but I'm trying to devise a plan because according to the trends, the case will probably be adjudicated before my fiance finishes his military service (he also has no internet connection, no use of a computer, and receiving mail is sketchy). So as of right now I'm planning to send Packet 3 to the Turkish Embassy myself. It's not ideal, but since interviews are being scheduled 3-4 months after these forms are received- if I have the option to move the process along a little bit quicker, I'll take it (rather than waiting until he gets out of the army to mail Packet 3). So it looks like I will completely fill out the form online and then mail it to him, have him sign it, and mail it back. I'll also have him sign a few copies of DS-320 and then I'll fill it in by hand.

For packet # 3 you are also going to need to send his current and archived police records from every city he has lived in since the age of 16, a certified birth certificate( there are two kinds.. he needs the one that lists every member of his family) WITH certified translations to English for both of these.

I can't remember.. is he from Istanbul? Some of the translation places are going to tell you that you need to get the translations notarized, some will not. The translations may or may not need to be taken to the district governors office for their stamp as well.

Some people I talked with said they didn't need it notarized, but needed the governors stamp.. tried to get the stamp, but since he didn't have the form notarized the office wouldn't stamp it, so had to go back.. get it notarized at the translation office and then get the district governors stamp.

I have talked to people who didn't get either one ( notary or district governors stamp) and had no problems.. others had packet # 3 sent back because they didn't have them. Safest bet is to get both.

If he is still going to be in the military, his family will need to get all these things for you and mail them to you for packet # 3.

The birth certificate and police records themselves don't cost much. The translations are going to be between 60 and 80 lira.. and the notarization is going to be between 80 - 100 lira.

One more thing.. packet # 3 must be sent UPS. They will not accept it any other way.

Oh, I just remembered a few more things.. there is going to be another form that needs to be completed and it will be emailed with the instructions they send you for packet # 3. I would have to look in my files, but I am pretty sure he has to sign that one as well.

I was able to get this form, from someone else, before I got the instructions for packet three.. so PM me and I can get your email address and send it to you. That way you don't have to wait for the packet # 3 info to get the form. Maybe that one didn't need to be signed... will look in my computer files tomorrow when I get to work.

You mentioned he did not have access to a computer. The instructions for packet # 3 are going to come by email. We both received the email, but I have talked to other people where only one of them got it. In some cases it was the beneficiary, in some cases it was the petitioner. I think most people said they both received it.. but if they only send it to him, does he have any way to check his email at all?
