Timeline

Hi, I just have received my work authorisation card with my name changed but I still have the SS card with my maiden name. What do I need to show to the SS office in order to make the change - would the the work ID and passport be enough?

The K1 precess took us 9 long months.

AOS

09/25/09 - POE: JFK

10/05/09 - married on K1 visa

11/23/09 - filed for AOS, EAD, AP

12/21/09 - Biometrics

02/01/10 - received work permit and adv parole

02/11/10 - Card production ordered!

02/23/10 - Green card received

To report a name change, fill out an Application for a Social Security Card (Form SS-5). You can get the form by visiting www.socialsecurity.gov on the Internet or any Social Security office or by calling Social Security’s toll-free number, 1-800-772-1213.

You must show us a recently issued document as proof of your legal name change. Documents

Social Security may accept to prove a legal name change include:

* Marriage document;

* Divorce decree;

* Certificate of Naturalization showing a new name; or

* Court order for a name change.

If the document you provide for a legal name change does not give enough information to identify you or if you legally changed your name more than two years ago, then you also must show us two identity documents including:

* One document in your old name; and

* A second document with your new legal name.

In addition to your name, these documents also must contain identifying information or a recent photograph.

If you are a U.S. citizen born outside the United States and our records do not show you are a citizen, you will need to provide proof of your U.S. citizenship. If you are not a U.S. citizen, Social Security will ask to see your current immigration documents.

The new card will have the same number as your previous card, but will show your new name.

C. Procedure - Immigration Document as Evidence of Legal Name

When issuing immigration documents, the Department of State and DHS issue them in the person’s legal name. The legal name is also generally the name in which the foreign passport was issued.

When an alien applies for an SSN card, presume the name on the immigration document is the legal name unless the applicant presents evidence of a legal name change (e.g., marriage) that occurred after the immigration document was issued.

In cases where an alien applies for a replacement SSN card and submits an immigration document showing a name that is different from the name on the prior Numident record, accept the immigration document as evidence of the legal name. In these cases, the prior SSN card may have been issued in a name that was not the NH’s legal name.
