Thank you to all who replied. I did get the RFE letter today. The Vaccination record is incomplete and was not signed. The Vaccination Worksheet does in fact state "incomplete" on it.

I will have my wife go to a civil surgeon next week to transcribe the vaccinations on the proper form and to receive whatever is lacking.

I don't know if this will be the only RFE letter I receive, but I hope they included everything on it.

We had 2 RFEs....

1 because we had filed an outdated version of form I765 and 1 because we had failed to supply evidence of I864 joint sponsors US citizenship.

The 864 one we had would not apply to you if you only have 1 sponsor.

However, they say that the most common reason for RFEs is forgetting to sign forms.

Good luck. I remember waiting for my RFE letter, it is nerve wracking.

It will likely be cheaper to get the vaccinations completed before visiting the CS. If you can't go through your regular doctor, check at the health department.

I went ahead and scheduled a regular doctor appointment for the remaining vacinnes and found a civil surgeon to transfer the vaccines over to the I-6 whatever it was.... You definately have to shop around for the best price on civil surgeons. One wanted to charge me $215, another one closer to my work is charging $42.50 for the same thing. And what's up with the $215, cash only, no credit cards or checks? Seems shady to me. Should I report it?

You could try to report it. Truth is that I don't think anyone does anything about it. The best way I can see to get back at them is to share information about CS's who DO provide good service for a reasonable fee. You can add the info of the $40 transcription to the thread we've got going here:

It happens to me too because the form I-765 that I filled up I didn't noticed outdated and its already save to my AOS file and I just printed out when my papers was complete and the form came back and it says application rejected becuase of the outdated form so I send it back and then I got an RFE for they need a proof that i had a pending I-1485 so I send it again finally I got approved. I applied may AOS last week of October and got my interview last January 29, 2010 and I got approved February 1, 2009 so I'm waiting for my GC to come at the mail within 30 days.
