Yea, as I mentioned before we payed $50 bucks, my husband didnt do any shots, because in Serbia they already gave him mmr and a Tetanus, so he didnt need the "TB shot". I just told the Dr. that he needed to fill out the form, and he did, but like u said we did wait in the office for at least 30 minutes for them to like print the sheet, a lil waste of time I say. He did take cash in the office, and I gave him 50 bucks and he gave me the envelope sealed. Like I sad, my hubby already had his shots done, so maybe thats why it was 50 bucks. In serbia, they give mmr, and tetanus when they do the medical for the K1 visa, which was still valid when he did his vaccination supplement !!!

and he didn't even glance at us as he walked by and made us

wait for about 70 min. on the second day. As I said he got it

done but I had to take 2 days off for something that should

have taken 30 min. She also had MMR & tetanus; he was yanking our crank.

Vaccination records required if 18 years old and under

If 19 or older, vaccination records are useful but not required

Fee includes blood testing for syphilis; tuberculosis skin test (PPD) or

chest x-ray; vaccination for Tetanus; and completion of I-693 form *

If you are given a tuberculosis skin test (PPD) by us, you must return to our

office 48 to 72 hours later to have the site on your arm examined by us before
