Hey guys,

I am helping a friend file for his adjustment of status, but I had a few questions about the application packet.

We've filled out a I-130 form with all its required documents. And, a form I-485 with all its required documents.

In addition, we've filled out a form I-864, and I-765, but I am confused about I-693. Are we suppose to get an appointment beforehand, get this form filled out by a doctor, and include it in the application? or is this for later when we get an appointment letter sent to us?

Another thing that I wanted to make sure is that are we suppose to arrange ALL the forms in to one packet and send it together? or send them separately? I was of the impression that I should send them together, and send them to the address at which I am suppose to send the I-485. am I correct about this?

also, can it be one big check or am i suppose to send three different checks (I-130, I-485, and biometrics)?

thank you.

To each is a goal to which God turns him; then strive together (as in a race) Towards all that is good.

Wheresoever ye are, God will bring you Together. For God Hath power over all things.

Yes, you get an appointment with a Civil Surgeon. They're listed on the USCIS website. Do a search to find the ones in your area, and phone around to get a good price. Since you're filing an I-130, I assume your friend did not have an immigration medical done before entering the country and/or he entered the country more than a year ago, so he'll need a full immigration medical. Expect to pay 250-400 dollars for that.

The CS will do the examination, record the results on the form I-693, and seal it in an envelope. You include the envelope with your I-485.

You are correct: all of the forms go in one big envelope, and all go to the same place: the address where you file the I-485.

You just send one big check.

DON'T PANIC

This was the only thing I was unsure of in HeatDeath's response. I sent two different checks: one for the I-130 and the other for the I-485/I765. I know there are a few people on here who sent one check, but don't know if they ran into any issues or not.

Sending two cheques for the I-130 and the I-485 is fine. The biometrics fee is included in with the I-485 cheque or you could send it separately if you wish. USCIS just wants their money. You just need to make sure the amounts are correct and they're happy. Be sure to make the cheque(s) out to the Department of Homeland Security.
