Timeline

Hey Guy my wife and I are filing of AOS papers soon, but we are lost on the Civil Surgeon medical part. She had her medical done in Vancouver during our fiancee immigration two months ago.

Was her medical forwarded to the place where the process AOS which included her vacinations? If not when I contact a Civil Sugeon what am I precisely asking them what to do and what do I need to bring with me to the civil Surgeon ?

oh and I did read the guides there are just conflicting subject matter to everything I look up.

When your wife completed her medical in Canada, she should have been given a sheet with her vaccinations marked on it. DS-3025. If the vaccinations are complete, then you just need to get it transcribed by a civil surgeon. If not, complete them with your regular doctor or the county health department. The CS can usually do them for you - but for much more money.

The guides say you might not need to do the transcription if your vaccines are complete, but in recent months, people have been getting RFE'd for it even when they were complete, so, better to spend a couple bucks to be sure.

This guy in Atlanta will transcribe that onto an I-693 for $40! (or he has recently for another VJer)

The instructions state that for a K-1 you only need part 1 (info about you), part 2 - vaccination portion only, and part 5 (civil surgeon certification).

Send in the sealed envelope that the CS gives you along with a copy of the DS-3025.

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

That might be the best you can do - you also might try calling the medical place in Canada and asking if they could fax/email you a copy as it wasn't given to you at the time.

If you can't get the DS3025, then you'll have to do the best you can - get the vaccine booklet transcribed by the CS. I don't see how it could be that big of an issue, I mean either she shows the booklet to the panel physician in Canada who copies it and he copies that or he just copies the booklet himself, see? It's all on your honor that it's correct.
