The I-693 is filled out by the CS doing your original medical. The I-693A is a supplement form to I-693. It's filled out by the CS here in the US when one files for AOS.

Also it's more than just $13. For a TB skin test and X-rays I didn't need the cost break down was

TB skin test $25. Physician visit and consultation $75, X-rays chest, side and back $115.

The I-693A form is no longer accepted. The I-693 was revised 6/5/08 to include the vaccination supplement within the I-693 document. It was revised again 10/14/2009.

Something to note:

2.Reminder: On or after January 1, 2010, the results of the medical examinations must be recorded on the October 14, 2009 edition of Form I-693. Any previous editions of Form I-693 that are submitted on or after January 1, 2010 will be rejected.

Ummm no it's not. You are required to take form I-693 with you, and if you're a K1 applicant, only complete part 1, part 2 (the immunisation schedule) and part 5. My information comes directly from the instructions... and I also know from other peoples experiences they WILL send an RFE if all 6 pages aren't included (whether completed or not).

You are working on advice from over a year ago... Requirements have changed. Please take the time to read the instructions here: http://www.uscis.gov/files/form/i-693instr.pdf specifically page 4, Section III, 2. C, B which confirms the above statements.

The main point of contention being that several VJ members have reported their completed DS-3025 which is supposed to be accepted according to I-485 instructions, they have since been sending RFE's requesting an I-693 (completed in the manner I described above) be forwarded.

We did the skin test at the local health dept for 15 dollars. No one (CS) told us we had to do it, but we did bec others had received RFEs in previous months for not doing it. They thought it was weird since we had a clear xray, but we just wanted to be safe.

I had also speculated that this was the case, but actually, I haven't seen any evidence that the TB skin test is actually required. Has anyone reported that the skin test was specifically requested? Also, people have been recently approved without the skin test...Are there cases that you know of that we don't?

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.
