Medical Form when filing for AOS

I wanted to post this question in the UK forum rather than AOS to see what other UK filers have done. After having my medical at Knightsbridge, i was told i didnt NEED to have all the shots done with them on that day. So to save money i thought i'd go back home to the midlands and have my MMR jab there on the NHS.... so my DS3025 is incomplete in that on the day it was filled out i hadnt had my MMR booster.

Im now filing for AOS and i've heard that some people have had problems and got RFE's for incomplete medical forms. I have a letter from my nurse confirming the day I had the MMR jab with her, to accompany the DS3025. will that be enough or would anyone recommend getting another medical done over here in the states and using the form I-693??

thanks, Jo

When I had my medical in November, I wasn't up to date with my teatenus (really annoying, because the doctors told me I was fine, so I'd thought I was good to go) and was advised by the nurse not to bother with the vaccination at the medical due to costs.

I panicked when I got back, after reading that it's maybe not quite as simple as the nurse made out. I posted a topic about it on VJ, and got some helpful answers.

Hi Jo, welcome to the next step.

You'll need to get a CS to transcribe your vacs onto the I693. So many people from the nov/dec filers are getting RFE's for medicals. I had the CS fill in the appropriate parts and you don't need another medical. It seems it can be difficult to find a CS who will just transcribe without trying to get you to do a whole new medical. I found mine on the USCIS website.

https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

I also had to have a second MMR shot as she claimed it's a 2 shot booster. It cost me $45 for transcribing and another $65 for the shot.

Good luck

Amanda :)

i think i've found a doctor whose happy to simply transcribe my vaccine records... so am i right in thinking he only needs to fill out Part 2 Medical History, section 5: Vaccinations. is this correct? or does he still need to fill out the rest of the form I-693? gosh, so confusing!!!! thanks for the help!

This is for people getting medicals in the future.

If you should find you need another shot when you get to Knightsbridge, and you want to try to get it free from you NHS doctor...

The Knightsbridge office will hold your medical (they wait on the lab results anyway) and allow you to get your shots elsewhere like NHS. They will give you their fax number and accept a fax from your doctor showing the new shot you got. Then they will add it to your record before sending on the the Embassy. If you medical is well before your interview, then that would be an easy option. You can't add on any extra shots from a personal doctor to an AOS application. If it's not recorded by the Panel Physician on the DS3024, then it must be recorded by a Civil Surgeon on the I-693.

Better yet, everybody seems to need the same thing, so before your medical, get a MMR and Tdap or Td (or maybe DTP/DTaP/DT) and take a record of shots to Knightsbridge. Those last three are for babies, but that may be all your NHS doctors have. That's what my husband took. Earlier research led me to information that said the Tdap was developed for adolescents and adults because they sometimes had adverse reactions to the stronger baby versions. But the Tdap is a US drug and only on the market in the US. The difference in the versions (in US talk) is a capital letter means a big dose of that vaccine vs a lower case letter meaning a smaller dose.

D or d =diptheria

T or t = tetanus

P or p or ap or aP = anticellular pertussis (whooping cough)
