I'm sorry...it was for transcription of vaccination records to the I-693. I failed to ask the price for a complete medical since it didn't apply to our situation...and I was so relieved that the office staff actually knew what I was requesting when I called. We didn't have the DS3025...we only had my husband's vaccination record from the local health center in Jamaica, and his receipt for his medical exam in Jamaica. Hope this helps!

Thanks so very much for this Jawi! You may have saved us a whole lot of headache. We not at this stage yet but I'm already thinking the medical part of AOS is the killa for me. We have Den's vaccination record and his receipt from his medical at MoBay as well. I am saving this information right now!

Blessing on the rest of your journey!

When I went there (almost 3 years ago) the place was a mess.... the nurse was really rude... and they messed up on my paperwork.

But.... they're cheap... and they handle a lot of immigration medicals.

Old and Grumpy....But an American Citizen!!!

Just a WARNING, I was in a bit of a hurry and trying to finish the vaccination on a Saturday so I didn't check before going at the visit (on a Saturday he is open only from 8:30 - 10:30, and on Friday he closes at 1pm). The current price only for a vaccination transcript (part 9 in i-693) is $120 which I didn't expect and I felt a bit ripped off, considering that in Alpharetta I found much closer CS to my place that had a 150$ cost. In other words, it should be like

Civil Surgeon in Atlanta $120

Either way, the lesson is to always check before going, or at least get a quote before submitting your documents ... this is internet and everyone can post.

Hi,

How much is the total fees you have to pay for the medical examination? I am also in Atlanta. Do you know any CS who is really cheap?

Hi, I'm also in Atlanta - did you find a good, reasonably-priced CS in the end? Thanks!

Yes. I found one and got it done already :)

Great. Would you mind sharing the details of where you went, how much it cost and how long it took to get an appointment?

Thanks!

I am a Atlanta filer and got it done locally. Most of the vaccines were covered by my insurance. My civil surgeon charged $100 for the form.
