My fiancee arrived in the USA jan. 15, 2010. Does she have to do a new medical and vaccinations record before we send the AOS package?

Just need vaccinations to be transcribed to a 693 (and any new shots, if needed).

As long as you file the AOS before the 1 year mark on your K-1 medical.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

So just take her vaccinations card ( that's all she's got on her as far as her medicals is concerned) to the civil surgeons office and they'll handle the rest? Is that it?

Basically, yes.

I suggest that you try to find out the required vaccinations and if you could get some of them with your county health department. I got Tdap and vericella vaccines with them for 15 USD each shot along with the TB test for free. This might cost more with the civil surgeon.
